Position Description

Evaluate and treat patients as it relates to Primary Care and disease prevention.

Coordinate treatment plans and referrals when necessary to support continuity of care.

Counsel and provide patients with materials that support a healthy lifestyle.

Collaborate with other providers, hospitals, clinics and third party services (Home Care Agencies, Physical/Occupational Therapy & Rehab Facilities, etc).

Support clinic goals as it relates to quality improvement, patient engagement and community outreach.
